  • Bona Fide Resident of Makati City: Applicants must be actual, current, and verifiable residents of Makati.

  • Registered Voter in Makati: A crucial requirement is an updated Commission on Elections (COMELEC) certification or voter verification. This often entails being a registered voter in Makati for a specified period (e.g., at least two years). For minors applying as dependents, the voter certificate of their parents or legal guardian is required.

  • Filipino Citizen: Applicants must be Filipino citizens or hold dual citizenship.

  • PhilHealth Coverage: Applicants must have a current PhilHealth Member Data Record (MDR) showing its validity period.

    • If a valid MDR is unavailable, applicants may provide any of the following substitutes:

      • Certificate of Employment or Payslip (for employed individuals)

      • Updated Official Receipt (for voluntary PhilHealth members)
