1. Prepare the Required Documents

Gather the necessary documents, which typically include your birth certificate, two valid IDs (with your photo and signature), and proof of residency. Valid IDs may include your passport, driver's license, or other government-issued IDs.

2. Fill Out the Application Form

Visit the nearest SSS branch or download the E-6 form from the SSS website. Complete the form with accurate and updated information.

Once your documents are verified, you'll undergo a biometrics process, including fingerprint scanning and photo capture for the ID. The photo will be taken on-site.

5. Payment of Fees

Pay the necessary fees for the ID. The fee may vary, so it's best to check with the SSS branch for the current amount and payment methods accepted.

6. Wait for Processing

After submitting the application and completing the biometrics process, your application will undergo processing. The time it takes to receive your ID can vary, so be patient during this stage.

Once your SSS ID is ready, you'll be notified by the SSS branch. You can then return to the branch to pick up your ID in person. Make sure to bring a valid ID for identification when claiming your ID.

8. Verify Your Information

Upon receiving your ID, carefully review the information on it to ensure accuracy. If you notice any errors, inform the SSS personnel immediately for necessary corrections.

Step 3: Visit any of the following Philippine government agencies

Submit the requirements needed, the application forms and photocopies of IDs. Just wait until your papers get verified.

After your verification, you can now proceed to the photo booth for the picture of your ID, then have your signature and fingerprint. Then, wait your UMID ID for approximately 30 days via mail.

Step 4: You need to activate the card or else it will be useless. Your card will be activated 72 hours from the time you validate your fingerprints at the GSIS Gw@ps Kiosk. You can always ask staff there on how to activate unless you see yourself wondering how that is.

For further questions about UMID card. You can visit and ask in your nearest SSS or GSIS office.
